Are you standing comfortably,[link widoczny dla zalogowanych]
When Francesca Stuttard started secondary school, she is at agony due to her feet. "It was really bad,[link widoczny dla zalogowanych]," she says."I couldn't do any sport, I couldn't stand during my hour's singing lesson,[link widoczny dla zalogowanych], I could not even day my friends because I could not walk very far. And I had to wear school shoes or lace-ups all the time."
Her problem was a common one: flat feet. In her own case,[link widoczny dla zalogowanych], a floppiness within the rearfoot caused her feet to pronate (fall inward); this was compounded, because it often is, by short leg muscles. Consequently, her feet hurt so much that her mother remembers her crying after taking any exercise whatsoever, except swimming.
Children in Francesca's position will generally be referred by their GPs for an orthotics specialist who'll design inserts for his or her shoes to adjust the positioning of the feet, or to a physiotherapist. However when I told Lewis Stuttard, Francesca's father, that the kids concentrating on the same problems often described as growing pains happen to be down both those routes, he finished my sentence: "Don't tell me, neither orthotics nor physiotherapy worked. They frequently don't. Francesca had had many years of both. She also had stretching exercises and electronic muscle therapy, but there was no option to surgery."
Stuttard, like any father,[link widoczny dla zalogowanych], can talk feelingly about his 13-year-old daughter's ailments. But when you are looking at sorting out her foot problems, he has a major advantage. He's a consultant podiatric surgeon who runs using both private and NHS patients in the Highfield Hospital in Rochdale. Much more to the point,[link widoczny dla zalogowanych], he completed his training in Seattle where novel methods to problems such as Francesca's have been employed for yesteryear Fifteen years. There he learnt to do a surgical procedure known as subtalar arthroereisis, which works on 90 per cent of those cases that don't respond to orthotics or physiotherapy. He wanted Francesca to achieve the operation.
A stent, a little titanium plug,[link widoczny dla zalogowanych], is inserted in to the sinus tarsi,[link widoczny dla zalogowanych], the space between the talus (ankle bone) and calcaneus (heel bone), in order to keep that space open. The operation is conducted under local anaesthetic and involves only a 1cm incision using an X-ray for guidance. In 99 per cent of cases,[link widoczny dla zalogowanych], there isn't any adverse reaction however it can be simply reversed and recovery involves nothing more than bandages for a few weeks.
2 yrs ago, when Francesca's problems became acute, few surgeons within this country knew how you can insert these stents. Medical ethics prevented Stuttard from operating by himself daughter, so the family took a holiday in the usa where she had the operation.
A few week later, Francesca returned to school inside a wheelchair,[link widoczny dla zalogowanych], with her legs in plaster because she had also undergone another operation to possess her calf muscles lengthened by making a cut at the back of the lower limb, and releasing muscle casing. After a month, she was back on her feet, capable of taking singing lessons again, and go shopping with friends.
These operations are only just being introduced in the UK, and Stuttard wants to spread the term.
"When I started in this profession Two decades ago, feet were considered unsexy," he says, "the bottom from the pile. However the running boom has elevated their whole status: in Seattle, feet were viewed as engineering marvels, with their 26 bones, 19 muscles, 30 joints, 115 ligaments and 250,000 sweat glands. But,[link widoczny dla zalogowanych], due to the foot's complexity, surgery was, until recently,[link widoczny dla zalogowanych], only a last resort because the outcomes were poor. Now training and equipment have improved, and so do outcomes."
Stents are ideal for adults who have some mobility within the joint, and kids older than seven. The procedure is available on the NHS, although the guidance from National Institute for Health insurance and Clinical Excellence is that patients ought to be spoke of alternatives since there is, up to now, insufficient evidence on long-term outcomes.
It might be even more desirable to avoid the requirement for such procedures, perhaps by looking into making children operate straight and wear sensible shoes. Sadly,[link widoczny dla zalogowanych], Stuttard says, it's not that easy. "Very pointed toes or high heel shoes aren't good for feet, and ballet pumps that offer no support should not be worn constantly. But any shoe is alright it doesn't have to be expensive as long as it is wide and long enough, and provides support. Shoes aren't generally the problem: the main reason for most foot problems is biomechanics.
"If there's no 90-degree angle between your foot and the leg, the body compensates. What happens in the foot has consequences throughout the body. When the bodyweight isn't based on feet correctly, misalignment within the legs, hips and back results, leading to joint pain as far as the neck. If these problems aren't treated, gradually mobility is lost and arthritis often results."
Stuttard takes off his shoes as one example of, first simulating the slumped posture of someone with flat feet,[link widoczny dla zalogowanych], then curling his feet therefore the weight is redistributed to the sides. As he achieves this, his legs, pelvis and back all change position, leaving him standing taller and straighter. Soldiers with flat feet, he says, often suffer serious problems because intense exercise creates pressure about the bones. Many need to leave the Military simply because they develop medial stress syndrome (pressure on the tibia causing pain in the front of the calf).
Flat feet run in families: both Stuttard and his wife, Rita, will be to blame for his or her daughter Francesca's problems because he has short calf muscles,[link widoczny dla zalogowanych], and Rita's feet pronate. Francesca has suffered due to their genes but forget about. With pride, she hands over an envelope. It has all of the rosettes she has won in the last two years for netball, tennis, hockey and cross-country running, that she competes nationally for the North of England Air Cadets.
